This policy exemplifies the worst of what's wrong with the way we as a culture approach fitness, health, and wellness. Like so many other institutions jumping on this bandwagon, Lincoln makes a fatal error in logic: It conflates weight with fitness.

So heavier freshmen at Lincoln are assumed to be "unfit" and "unhealthy." Just as bad, thinner freshmen are given a pass on the fitness course, the assumption being that they don't need it because they're not that fat.

If fitness and good health are the goals, shouldn't everyone have to take the course?

Some people object to the fitness course on libertarian grounds. Not me. I think good health and fitness is part of what we should be teaching children and practicing ourselves. But threatening or punishing larger people because of their size is not a useful strategy. As a researcher from the National Institutes of Health said recently, the number-one cause of obesity in this country is dieting. Programs like this buy in to the fat-is-unhealthy mindset. They also buy in to the thin-is-healthy mindset. By conflating weight with disease they do everyone a grave disservice.

My own school, Syracuse University, is looking at "wellness" programs right now. I hope I don't have to write a piece like this about SU someday.

Because I don't care. I find the opinions and actions and health of models irrelevant to eating disorder activism. What DOES concern me is a tragic over-valuing of the role of the media and a conflation of what that world does to healthy people (not good) and the symptoms we see in the mentally ill.

Modelling surely does attract and exploit very tall thin pretty people, and does cause slightly less naturally tall thin pretty people to do unnatural things to compete with the former, and without a doubt attracts lots of people whose mental illnesses make those unnatural behaviors a resume-plus.

But the attraction to modelling, and to staring at and comparing oneself to models, is a symptom. That fascination is a symptom of a silly and self-destructive part of modern life for otherwise mentally healthy people. The compulsion, however, is a symptom, for some people of mental illness. Symptom, and not cause.

Home improvement magazines and household cleaner advertisements do not cause OCDs. They attract those WITH these brain patterns. For the rest of us, we just struggle to keep up appearances or suffer fits of dismay at how we fall short. People whose brains are healthy are usually able to distinguish a sales pitch from realistic house management.

When Martha Stewart shares her tips for homemade lemon cleanser we all know she has a staff to think this stuff up and do it for her. OCD advocates don't call for her head because of her unreasonably tidy TV studio. When a model professes a devotion to thinness we should know that that is what she is paid to do and be and that this set of values are the only reason she blights our news feeds and screens. The fashion industry doesn't cause eating disorders, it exploits them and the mentally healthy who try to live on green salads and cleansing fasts in pursuit of thinness.

We should refuse to care what purveyors of self-mockery say or do and expend our outrage on the maddeningly unhelpful comments of those who really should know better: those speaking of eating disorders as if they are just incorrect thinking and mistaken priorities. This approach strikes me as terribly cruel to patients and undervalues THEIR heroism and hard work to recover.

Over-exercising and diet restrictions can become an obsession

Josh Pittman, freshman integrated social studies major, realized that to win his high school wrestling matches he had to reach a lower weight class.

To cut weight, Pittman began an obsessive cycle of radical calorie restriction and exercise, and soon the 5-foot-9 Pittman went from a slim 135 pounds to a dangerously low 112 lbs.

"I didn't think I was that thin. I was anorexic," Pittman said. "I became obsessed; it was a way of life and the only thing that was important to me."

Over-exercising and not eating wore Pittman down to a point where he fell asleep in classes, frequently passed out and was agitated all the time.

Josephine Glover, a licensed clinical counselor at Malone University, said once the eating disorder pattern has been established, it becomes an addiction and takes on a life of its own.

"The person begins to have a relationship with the eating disorder instead of real people," Glover said. "All their time is spent centered around food; it becomes an obsession."

Glover helps manage the Jubilee Center, a Canton eating disorder support center where the general majority of patients are college-aged.

Some studies have shown that as many as 50 to 60 percent of the college population has an eating disorder. Kent State students don't seem to be exempt.

Marie LePage, the undergraduate officer coordinator of psychological services in Kent Hall, said an increasing amount of students are going in for treatment.

"College is a time of greater stress, and it can be a way to deal with the stress," LePage said. "For many young people, it is truly their first time on their own. They are suddenly thrown into this world of greater freedoms, mom and dad aren't there to overlook their every move."

Other factors that contribute to eating pathology are genetics, peers, family and the media. LePage said eating disorders are a culture-bound disorder and largely influenced by societal pressure. The West is highest among many cultures cultivating the "thin is in" mindset to the extreme.

Sarah Yonkof, freshman exploratory major, said students are competing to look good.

"I feel like you're only as good as your size," Yonkof said. "Guys treat you better if you're skinnier - the attention is always based on your looks, not how smart or funny you are. No matter what your education, social status or money situation, people judge you on your weight - and I think it's wrong."

But women aren't the only ones feeling the pressure to "measure up."

"A big misconception is that guys aren't affected too," Pittman said. "Guys get self-conscious, just like girls. I remember, starting college, I gained some weight and got real depressed because I hated how I looked and felt."

While Pittman was able to reduce his disorder after quitting wrestling in high school, he said it still affects his life in college - though not as severely. He said if he's not careful, he begins a downward spiral of excessive exercise and strict calorie restriction - sans beer and snacks.

Glover and LePage agree recovery is possible with help from the right people. Pittman advises people suffering from the disease to get support.

"All I can say is there is nothing to be ashamed of," Pittman said. "Everyone has problems, whether they let people know or not. If this is your problem, seek out the help and get it; it's not worth your life."

  • Dr. Rain Carel had 25 of the teenagers participate in yoga classes for two months.
  • While the activity had no effect on their weight, taking the classes reduced their obsessive concerns about weight, food preoccupation, and other measures.

It's a concrete example of distorted thinking. We can get fooled (by advertisements, for example) into wanting something even though it isn't healthy for us. But those with eating disorders are willing to make a conscious choice to sacrifice health for weight loss or weight maintenance.

    But, she's not. That's what I learned last week, when I was invited to sit on a television panel to talk about Barbie's ankles. It seems French shoe designer Christian Louboutin told Women’s Wear Daily magazine that Barbie's ankles were too "fat" to wear his shoes.

    Louboutin, who is designing three special edition Barbies, is reshaping her gams (slimming her ankles) in order to wear his shoes. Mattel, Barbie's maker, reports that the Louboutin controversy (known in the media as "Barbie Has Cankles!") is simply a misunderstanding: "My dear friend, Christian loves my ankles. It was my arch that he wanted to give a little lift to, so I can rock those high heels."

    But, you see, one of the wonderful perks about being a shoe designer is that you actually design shoes!     It's not a case of people getting the stuff on some kind of black market. It's people buying it over the counter in Ireland. According to licensing conditions, pharmacists in Ireland can't sell Alli to anyone who's of normal weight or underweight. But eight out of 20 pharmacies sold the drug to a researcher who is well under the "overweight" threshold.

    Not a good situation. But I wonder: How is a customer supposed to verify that his or her body mass index is in the proper range to be given the drug without a prescription? Must there be a scale in each pharmacy? I can't imagine a good system.

The House of Representatives passed a health are reform bill that includes a health insurance plan offered by the federal government.

    The new bill, the Affordable Health Care for America Act (H.R.3962), replaces the previous bill (H.R.3200) originated in the House. H.R.3962 passed by the slimmest of margins, but passed nonetheless.

    What would this bill mean for you if the Senate passes similar legislation and it becomes law? If you don't have insurance coverage now, you probably will. If you're a low-income American, you may qualify for a subsidy to help you buy health insurance. More employers will be providing coverage for their employees.

    Perhaps more importantly, the bill prohibits your health insurer from refusing coverage (or charging different rates) based on your medical history. This could have an impact on those with eating disorders, since medical problems are common.

    I haven't seen anything about whether you should expect to see your insurance rates rise under these conditions. Insurance companies charge high-risk customers more and low-risk customers less. If these charges have to be the same across all customers, you may see the rates on your current policy rise (or fall, if you're a high-risk person). Unless, of course, you end up with the public option.

While I'm all for encouraging healthy eating and exercise, this is not the goal of the show. Instead, contestants are put on low calorie diets and required to engage in extreme exercise, a combination that is simply unsustainable. A recent issue of Us magazine profiles three former contestants:

    1) Hollie Self: Self is reported to eat 1,200 calories a day, run four times a week (up to 20 miles per run) and engage in strength training three times a week. Self completed the NYC marathon (5:08) and is training for a Half Ironman next year. For the uninitiated, that's a 1.2 mile swim, a 56-mile bike, and a 13.1 mile run (yes, all in one day). But, on a 1,200 calorie/day diet?

    2) Alexandra White: It's reported that White eats 1,300 calories a day and works out for four hours a day, six times a week. This type of overtraining is likely to lead her to burnout and injuries and again, isn't sustainable on 1,300 calories a day.

    3) Nicole Brewer: Brewer reportedly eats 1,600 calories a day, as she trains for the Philadelphia Marathon and teaches spinning and strength training classes.

    My concern about these lifestyles is that they're unrealistic--the general public is led to believe that it's possible a) to function long-term on low calorie diets b) and to do that while engaging in frequent, rigorous endurance exercise. For most people, it's a set-up, at best, for failure and low self-esteem (If they can do it, why can't I?), and at worst, for illness and malnutrition.

    At least, according to researchers at Laval University in Quebec, who have just published the results of a randomized trial comparing Health at Every Size with other food-related interventions.

    The study involved premenopausal women considered overweight or obese, who spent time either in an HAES group, a support group, or a control group. A year after the trial had ended, the women in the HAES group had less "situational susceptibility to disinhibition," meaning they were less likely than women in the other two groups to overeat when triggered by stress, abundance, and other external situations. Women in both the HAES and support groups ate less in response to perceptions of hunger.

    I'm a little irked that the ScienceDaily writeup of the study refers to HAES as a "new weight paradigm." But hey, it's still good news, and the beginning, I hope, of more studies looking at the benefits of HAES. Now, if only I could get these study results to the last group of doctors I spoke to . . .

    Objective: The present review addresses the outcome of bulimia nervosa, effect variables, and prognostic factors. Method: A total of 79 study series covering 5,653 patients suffering from bulimia nervosa were analyzed with regard to recovery, improvement, chronicity, crossover to another eating disorder, mortality, and comorbid psychiatric disorders at outcome. Forty-nine studies dealt with prognosis only. Final analyses on prognostic factors were based on 4,639 patients. Results: Joint analyses of data were hampered by a lack of standardized outcome criteria. There were large variations in the outcome parameters across studies. Based on 27 studies with three outcome criteria (recovery, improvement, chronicity), close to 45% of the patients on average showed full recovery of bulimia nervosa, whereas 27% on average improved considerably and nearly 23% on average had a chronic protracted course. Crossover to another eating disorder at the follow-up evaluation in 23 studies amounted to a mean of 22.5%. The crude mortality rate was 0.32%, and other psychiatric disorders at outcome were very common. Among various variables of effect, duration of follow-up had the largest effect size. The data suggest a curvilinear course, with highest recovery rates between 4 and 9 years of follow-up evaluation and reverse peaks for both improvement and chronicity, including rates of crossover to another eating disorder, before 4 years and after 10 years of follow-up evaluation. For most prognostic factors, there was only conflicting evidence. Conclusions: One-quarter of a century of specific research in bulimia nervosa shows that the disorder still has an unsatisfactory outcome in many patients. More refined interventions may contribute to more favorable outcomes in the future.

    I was interviewed this morning for the SU campus newspaper about Project BODYTALK. The reporter asked me an interesting question: Do I think that the act of telling your story around food and body image is healing? Is that what I hope to accomplish through this project--healing for the people who tell their stories?

    My answer: Absolutely. But I also know that healing--in whatever form--will also happen through listening to these powerful stories.

    To that end, I'd like to collect as many commentaries as possible. You don't have to have an eating disorder or serious body image issue to tell your story--in fact, I'd like to hear from people who feel good about their bodies, too!
